SPECIAL PERMISSION FORM

Please initial each of the following (below) that you, as parent/guardian, agree to. You may pick as many (or few) as you wish. The purpose of this form is to allow you to decide at what level you wish your child to participate in Internet-related classroom activities.

Please discuss Internet safety with your child at the beginning of each school year.

_____ Parent or guardian has completed the standard Assumption Internet Usage permission form for each child.

_____ Parent or guardian agrees to allow publication of student’s work on the Assumption website, knowing that granting such permission removes any copyright claim of the student, and requires that the student NOT reproduce previously copyrighted material.

_____ Parent or guardian agrees to the use of the student’s FIRST NAME ONLY in crediting of work published or other accomplishment. Teacher or grade may be used in conjunction with the use of the first name.

_____ Parent or guardian agrees to allow pictures including the student to be published on the Assumption website. 


Please sign below, indicating that these are the times you will or will not approve for your child in regards to the Internet and Assumption Computer Lab. Usage of the Internet will be withheld from any student for whom there is not a signed permission form on file.
